Complete, sign and return the Emergency Health Information Sheet.

Inform the School Nurse of any health/medical changes including new Medications.

Bring any prescribed medications and doctor’s orders your child will need at school before the first day, especially Epi-Pens.

Please be certain to turn in your child’s state required physical and immunizations form for grades K, 4, 7 & 10.
