Information for Residents

In order to register your student in the Rockland Public School District, the following forms must be completed and returned to the main office of the school which your child will be attending:

  • Registration Forms
  • Legal Birth Certificate with Raised Seal
  • Health Record, including Immunization Record
  • Proof of Residency
  • Transportation

School Choice Applicants

The Rockland Public School District accepts students under the School Choice Program from neighboring towns on a space available basis. Enrollment may be requested at any Rockland school if it has been declared by the Superintendent that a vacancy exists. The following documentation is needed to process a School Choice Application:

  • School Choice Application
  • Student Record
  • Health Record, including Immunization Record
  • Individualized Education Plan or 504 Accomodation Plan, previous and current (if applicable)
  • School Discipline Record, and
  • Legal Birth Certificate with Raised Seal.

After the following information is reviewed and the student is approved for School Choice admission, the applicant must fill out:
